Lifeline in a Bottle

This is the United Relief Foundation’s program to give those who would like the extra security of knowing their vital medical information is available to emergency personnel upon arrival at their home.
“Lifeline In A Bottle” is a program in which you are supplied with a prescription bottle and a form that the recipients fill out with their vital medical information.

The form is then placed in the prescription bottle and placed in your refrigerator.

Two stickers (one placed inside the front door and the other on the outside of the refrigerator door) are also supplied that lets emergency personnel know you are participating in this program and that they can retrieve your medical information if you are in any way incapacitated.

Having this information can make a difference in Save
a life that otherwise would not be possible without this crucial information.

If you are interesting in this program please contact us and we will be make sure your city/town is participating and if not we will attempt to make arrangements for your city/town to start participating in “Lifeline In A Bottle”.
